This notice describes, in general terms, how health information about an individual may be used and disclosed by the clinic and how that individual can access their information. The clinic's final, legally reviewed notice is provided in writing at the first appointment and takes precedence over this summary.
Health information may be used to provide services, to coordinate with other healthcare professionals involved in a person's care, and for internal operations such as scheduling and quality review.
Except where the law requires or permits otherwise, health information is disclosed to family members, employers, courts or other third parties only with the individual's written authorization. Authorization may be revoked in writing at any time.
Records relating to substance use services may receive additional protection under federal and state law. The clinic will explain those protections and any limits in writing before information is shared.
